Our Referral Pathway
Referrals to the Psychiatry of later life team can be made by your G.P. or any medical Doctor involved in your care.
When referred to our team your care will be discussed at our weekly team meeting. A member of our team will make contact with you to arrange an assessment that works best for you.
Our Assessments
- Initial Assessment
Initial assessments may take approximately one hour. You may be invited to a clinic appointment or a doctor from our team may visit you at home or where suits you best to discuss any mental health concerns you may have. Family and any loved ones are welcome to be present at this time if this suits you. - What To Bring
It may be helpful to bring to your assessment details of your medical history, mental health history, medication list and details of your G.P., pharmacist and any specialist Doctors involved in your care if these are easily available to you at the time. - Your Plan of Care
Following your assessment, a member of our team will work with you in collaboration with the team to develop an agreed plan of care to meet your needs. The duration of time the team will work with you will be dependent on your care needs. - Follow-Up
We will update your G.P. and referring Doctor on the outcome of the assessment and any changes to your plan of care. - In-Patient Care If Required
